For example, you could ask that we not use or disclose information about a surgery that you had. We are not required to agree to your request.
- You have the right to request an "accounting of disclosures" (that is, a list of certain disclosures Tualatin Valley Physical Therapy, LLC has made of your health information).

Complaints:
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. You will not be penalized for filing a complaint.

We reserve the right to update or modify this Privacy Policy as needed to reflect changes in our practices, legal requirements, or technological advancements. Any updates will be posted on our website and become effective immediately upon publication.
